The following outlines LifeQuest's fees schedule. However, since medical requirements differ for each individual or couple, your physician at LifeQuest will make a point of clarifying applicable fees when you visit the centre. Please feel free to contact us with any questions you might have. IVF FEESOHIP Coverage: OHIP will cover IVF cycles for patients with bilateral tubal occlusion (blocked fallopian tubes) only, for a maximum of 3 cycles. A treatment cycle for any diagnosis other than bilateral tubal occlusion is uninsured.
The remaining fees are the same for both Insured and Uninsured patients:
Out-of-country patients $1000 for Insurance
Gestational Carriers or Donor Ovum Recipients (Uninsured)
OUR FEES POLICYPlease be aware that the payment for your treatment cycle must be made in full on the day of Suprefact teaching. All fees are due in full by Day 0. Please note, all fees are subject to change. LifeQuest fees are in Canadian funds. Some services are covered by provincial medical or private insurance plans. It may be possible to save money by insuring the costs of your treatments. We suggest that you speak with First Durham Insurance & Financial. Visit their website for more information.
